What is Family Advocate Connections Team (FACT)?
FACT is not a program but, rather a personalized plan and strategy to help individuals and families who have complex needs receive early access to coordinated services to resolve personal concerns and family issues.

This strategy, and our efforts to deliver it, will be based on a “whole family “approach. This is listening to the needs of the families, treating them as partners and to harness their own resourcefulness to build supportive community networks
